Virtual Direct Primary Care

Our virtual care is not a teledoc or ask-a-nurse service. Instead, it's personalized complete care with your primary care provider from the comfort of your home, or wherever you are.

Ages 18-64

Terms & Conditions

  • One-time $99 enrollment fee

  • You must live outside of a 50 mile radius from our clinic.

  • Enrollment fee is waived with annual payment 

  • Minimum 3 month commitment 

  • Patients are billed monthly and may discontinue their membership after a three month minimum commitment with 30 day written notice of cancellation.

  • Re-enrollment fee after cancellation is $250.

  • Virtual DPC (VDPC) Memberships are available only to residents of  Texas. By enrolling in VDPC membership, you agree that you are a resident of  Texas.

  • We are currently unable to accept any Medicare patients at this time due to certain Medicare guidelines. However, we anticipate being able to accept Medicare patients in the near future. If you are interested in joining and have Medicare, or will have Medicare within the next 6 months, please contact us at info@luminoushealthandwellness.com to be added to our Medicare wait list.

  • Dr. Medina Galvan does not prescribe narcotic medications or new prescriptions for benzodiazepines, or certain other medications deemed inappropriate under Dr. Medina Galvan's sole discretion.

Included in Membership

  • Unlimited video and phone visits

  • No copays or deductibles 

  • Discounted lab work and imaging that are local to you

  • Virtual urgent care for acute sick visits

  • Chronic medical care management (ie. Blood pressure, cholesterol, diabetes, mental health)

  • Extended visits

Not Included in Membership

  • In-House procedures

  • Pap smears

  • EKG or In-House Testing (though we can order this to be done locally to you in some cases)

  • Sport Physicals

  • Some urgent complaints, like chest pain or severe abdominal pain with fever, must be evaluated in person to make a safe and proper diagnosis.

 

All of these services are available with our full-service membership plan.
