DOT Medical Exam

A DOT physical is an exam for truck drivers holding or applying for a commercial license (CDL). This evaluation verifies the candidate's fitness to operate a vehicle safely, minimizing risks to themselves and others on the road.  The physical evaluates various aspects of the driver's health, including vision, hearing, blood pressure, heart health, and overall physical fitness.

  • Vision Screening​

  • ​Electrocardiogram (ECG), if warranted

  • Physical Examination

What to bring to your appointment

  • Complete the DOT Physical Exam Report Form before your appointment.  

  • Photo ID

  • If you wear glasses or contacts daily, please wear them or bring them with you for the vision test.

  • Copy of your most recent stress test (for heart conditions)

  • Copy of your latest sleep test result (for sleep apnea)

  • Bring a copy of your medical records to your appointment for instance a list of your medications and past lab work.

  • If you have a heart condition, you need to provide your DOT examiner with a note from your cardiologist stating that your heart health won’t interfere with your ability to drive safely.

Additional Information:

  • The DOT Medical Exam is NOT a replacement for your primary care physician's annual preventative physical. This visit is considered an occupational screening exam and not a therapeutic physician visit or evaluation. Therefore, we are not treating you for medical conditions at this exam.

  • A 24-hour notice is required to reschedule an exam. Exams canceled with less than 24-hour notice will not be rescheduled and subject to a $50 no-show fee.

  • There will be an up front $50 reservation deposit that will count towards your exam. Payment is due at the time of the examination. We accept cash and credit cards. We will not file with your insurance provider.
