Here are the next steps...

1. Check your email for an invite to create a patient portal profile.  If you don't see your invite,  please email management@luminoushealthandwellness.com

 

 


 


2.  Once you have logged into the patient portal, go under the questionnaires tab and fill out all the documents. 

3. Familiarize yourself with the portal and add in your medications, pharmacy of preference, upload your photo.

**As of 12/1/31,  I will not be using Spruce for after-hours texting, it will solely be used for video visits.**

4.  Bookmark the patient portal website (button at the top of this page).  

5. Download Spruce to your phone and make your account. This is our HIPAA-compliant app that will be used for video visits only.

6. Schedule your first DPC appointment using the patient portal. Select "DPC New 60 minutes".
