ABOUT THE EXAM
What is an immigration
medical exam?
An immigration medical exam — also called a green card medical exam — is required for all applicants filing to become a permanent resident or green card holder in the United States. The exam must be performed by a USCIS-designated civil surgeon and submitted using Form I-693.
Dr. Medina Galvan is authorized to conduct comprehensive immigration physicals and complete all required USCIS documentation for your case.

THE PROCESS
Steps to complete
Form I-693.

Schedule your appointment
Call our office or book online. Bring your passport, vaccine records, and any prior medical documents.

Exam, vaccines & labs
Dr. Medina performs the physical exam, reviews your vaccine history, and orders required lab work and any missing titers.

Lab results reviewed
Once results are received, Dr. Medina reviews everything and you will be contacted with next steps.

Sealed envelope issued
When all vaccines and labs are complete, you receive your sealed Form I-693 envelope to submit with your immigration case.
Important:
Do not open the sealed envelope. It must be submitted to USCIS unopened. The sealed I-693 is valid for 2 years from the date Dr. Medina signs it, as long as it is submitted within that window.

Valid government-issued ID
Passport (required), foreign national ID, or any government-issued photo identification.
Vaccination records
All prior immunization records — U.S. or foreign. This helps Dr. Medina determine which, if any, titers are needed.
Payment
We accept cash, debit, and major credit cards. Payment is due at the time of your visit.
